I was hoping you could add some way for servers that use custom entities to be controlled by spectators/dead. Or maybe add a convar that makes it like the old behavior that allows them to inhabit anything.

Would it be sufficient if custom entities could set a value like ENT.AllowPropspec = true, so TTT can check that and allow/prevent possession, or are you also dealing with entities that are not scripted?
